The main symptom of back pain is, as the name suggests, an ache or pain anywhere on the back, and sometimes all the way down to the buttocks and legs. Some back issues can cause pain in other parts of the body, depending on the nerves affected.

The fact that opioids are the most commonly prescribed back pain medication has put added pressure on the medical community to find different solutions. To that end, this past February the American College of Physicians released new guidelines on noninvasive treatments for lower back pain. The group’s primary message was that pharmaceutical options, and opioids in particular, should be treatments of last resort. In their stead, the authors suggested superficial heat as a well-studied method for relieving acute low back pain. They also found some evidence supporting the use of techniques such as acupuncture and spinal manipulation for acute and chronic pain—although it should be noted that several researchers have questioned whether these treatments are superior to placebo or sham therapies.

OVERWEIGHT: While it seems intuitive that LBP and excess weight could be related, a 2007 paper in the Journal of Rehabilitation Research and Development says the science cannot determine whether they are in fact directly related, under what circumstances they are related, how they become related, the strength of relationship (if one does in fact exist), and the impact of a change in one condition on the other. Complaints of low back pain are one of the most common reasons people visit doctors.[9][41] For pain that has lasted only a few weeks, the pain is likely to subside on its own.[42] Thus, if a person’s medical history and physical examination do not suggest a specific disease as the cause, medical societies advise against imaging tests such as X-rays, CT scans, and MRIs.[41] Individuals may want such tests but, unless red flags are present,[10][43] they are unnecessary health care.[9][42] Routine imaging increases costs, is associated with higher rates of with no overall benefit,[44][45] and the radiation used may be harmful to one’s health.[44] Fewer than 1% of imaging tests identify the cause of the problem.[9] Imaging may also detect harmless abnormalities, encouraging people to request further unnecessary testing or to worry.[9] Even so, MRI scans of the lumbar region increased by more than 300% among United States Medicare beneficiaries from 1994 to 2006.[11]

While this study used a pain population and quantified the degree of discomfort in different seating systems, it has some limitations. Firstly, the sensors used for determining the thoracolumbar and lumbar postures were placed on the skin surface. While the electromagnetic equipment and methods used in the current study are commonly adopted in biomechanics research [9], it is possible that a more direct method, such as radiographic measurement, would have yielded more accurate angular changes of the vertebral column [16,31]. The benefit of using the current equipment, however, was that it posed little to no risk to the participants, and it avoided any harmful effects of radiation that are associated with radiographic investigation [16].

Other problems may occur along with low back pain. Chronic low back pain is associated with sleep problems, including a greater amount of time needed to fall asleep, disturbances during sleep, a shorter duration of sleep, and less satisfaction with sleep.[24] In addition, a majority of those with chronic low back pain show symptoms of depression[13] or anxiety.[17]

Low back pain can cause a wide variety of symptoms and signs depending on the precise cause of the pain as reviewed above. Symptoms that can be associated with low back pain include numbness and/or tingling of the lower extremities, incontinence of urine or stool, inability to walk without worsening pain, lower extremity weakness, atrophy (decreased in size) of the lower extremity muscles, rash, fever, chills, weight loss, abdominal pains, burning on urination, dizziness, joint pain, and fatigue.
