Exercise. Regular low-impact aerobic activities — those that don’t strain or jolt your back — can increase strength and endurance in your back and allow your muscles to function better. Walking and swimming are good choices. Talk with your doctor about which activities are best for you.

The discs are pads that serve as cushions between the individual vertebral bodies. They help to minimize the impact of stress forces on the spinal column. Each disc is designed like a jelly donut with a central, softer component (nucleus pulposus) and a surrounding, firm outer ring (annulus fibrosus). Ramin wasn’t fully aware of spinal surgery’s poor rates of success when she decided to see a back surgeon for her own chronic back and leg pain nearly a decade ago. Then a freelance journalist, having just published a book on memory in middle age, she was frustrated and baffled by her own lack of progress, and her questions led her stumbling into a public health story that would take more than 600 interviews and eight years to write.

They might also be influenced by direct-to-consumer advertising from less reputable spine centers. “When they go see his surgeon and the surgeon says, ‘I’m sorry I can’t help you this. There’s nothing I can do for you,’ the tendency is to misunderstand that, and to think ‘You’re not smart enough. You’re not good enough; you don’t have the right high-tech whiz-bang tools,’” she tells Quartz, “and I need to keep looking. I need to find someone who is smart enough to do this.’”

Workers who experience acute low back pain as a result of a work injury may be asked by their employers to have x-rays. As in other cases, testing is not indicated unless red flags are present. An employer’s concern about legal liability is not a medical indication and should not be used to justify medical testing when it is not indicated. There should be no legal reason for encouraging people to have tests which a health care provider determines are not indicated.
Disc degeneration remains a key cause of chronic low back pain and the pain often persists despite surgery. NIH-funded basic science and preclinical studies are investigating molecular-level mechanisms that cause discs in the spine to degenerate, as well as protective mechanisms involved in disc remodeling that may diminish with advancing age. It also appears to reduce recurrence rates for as long as six months after the completion of program and improves long-term function. Transcutaneous electrical nerve stimulation (TENS) has not been found to be effective in chronic low back pain. Evidence for the use of shoe insoles as a treatment is inconclusive. Peripheral nerve stimulation, a minimally-invasive procedure, may be useful in cases of chronic low back pain that do not respond to other measures, although the evidence supporting it is not conclusive, and it is not effective for pain that radiates into the leg.
The Agency for Healthcare Research and Quality has identified 11 red flags that doctors look for when evaluating a person with back pain. The focus of these red flags is to detect fractures (broken bones), infections, or tumors of the spine.
