PELVIC TILT: Many health professionals believe that anterior pelvic tilt and increased lumbar lordosis indicate abdominal weakness and overly strong (or tight) hip flexors. But, according to a 2004 study in the Internet Journal of Allied Health Science and Practice, there is no relationship between lumbar lordosis and isometric strength of the trunk flexors, trunk extensors, and hip flexors and extensors. Several other studies have also had similar findings.
That’s easy—use them anywhere you need pain relief when sitting or lying down. Put on your bed when reading or watching TV. Put them in the car or truck. They are an added dimension when used in an office chair. They will help you to relax and remove pain when you are on a sofa or sitting in your recliner, airplane seat or even wheelchair. Neurological examination involves assessing sensation and function. The doctor may employ the pin prick test to determine if feeling is the same on both sides of particular parts of the body (eg, legs). Function, flexibility and range of motion are assessed while you walk, bend forward and backward (if able to), and during other movements. The doctor tests your reflexes too.
Type of chair: chairs with padding or built in lumbar support usually require thinner lumbar support cushions that provide support without forcing too far forward. Moreover, make sure you make a note of the dimensions of the cushion and compare it with your chair before ordering to make sure it will fit.
MRIs are not without problems. Bulging of the discs is noted on up to 40% of MRIs performed on people without back pain. Other studies have shown that MRIs fail to diagnose up to 20% of ruptured discs that are found during surgery.
Not all treatments work for all conditions or for all individuals with the same condition, and many find that they need to try several treatment options to determine what works best for them. The present stage of the condition (acute or chronic) is also a determining factor in the choice of treatment. Only a minority of people with back pain (most estimates are 1% – 10%) require surgery.
If infection, such as a spinal epidural abscess, is the source of the back pain, surgery may be indicated when a trial of antibiotics is ineffective. Surgical evacuation of spinal hematoma can also be attempted, if the blood products fail to break down on their own.

Imaging is indicated when there are red flags, ongoing neurological symptoms that do not resolve, or ongoing or worsening pain. In particular, early use of imaging (either MRI or CT) is recommended for suspected cancer, infection, or cauda equina syndrome. MRI is slightly better than CT for identifying disc disease; the two technologies are equally useful for diagnosing spinal stenosis. Only a few physical diagnostic tests are helpful. The straight leg raise test is almost always positive in those with disc herniation. Lumbar provocative discography may be useful to identify a specific disc causing pain in those with chronic high levels of low back pain. Similarly, therapeutic procedures such as nerve blocks can be used to determine a specific source of pain. Some evidence supports the use of facet joint injections, transforminal epidural injections and sacroilliac injections as diagnostic tests. Most other physical tests, such as evaluating for scoliosis, muscle weakness or wasting, and impaired reflexes, are of little use.

Mounting evidence suggests that exercise is among the best remedies because it is able to reduce pain, improve mobility and prevent future discomfort. “[Back pain is] not the only thing that’s been overmedicalized,” says back pain specialist Daniel C. Cherkin of the Kaiser Permanente Washington Health Research Institute.* “But it’s probably the poster child for how things can go wrong in terms of patient outcomes and cost to society.”
