Several risk factors have been identified for the development of LBP in individuals who are required to sit throughout the majority of their work day. These include prolonged muscle contractions [3,4], vibration [5], and sustained body postures. Postures outside of neutral are particularly troublesome [3,6-9] as they lead to prolonged low level muscle contractions [6] and changes in intervertebral disc pressures [8,10]. During sitting, the lumbar spine flattens and there is posterior migration of the nucleus [11]. The pressure on the disc increases [8,10] and there is increased passive strain on the posterior spinal elements [12,13]. The seated lumbar pressures may be minimized by maintaining the natural lordotic curvature [8,14].

While additional authors have investigated the effect of various support systems on bodily symptoms, much of this work has been performed on healthy individuals. Aota et al. [15] measured the biomechanical effects and comfort levels when using a lumbar support cushion that inflated from 0.5 to 8.0 cm thick in a continuous passive motion chair. Portable devices that do not account for the bulk of posterior pelvic soft tissue volume may push the lower body forward and distort the intended relationship between the seat pan features and the body [17]. The preferred degree of lordosis may be related to the pain state of the individual [17], in that comfort may be affected by the angular change as well as the interaction between the buttocks and the seat pan.

  1. February 26, 2018 • No, we’re not talking about squatting. We’re talking about a way to bend over that has nearly disappeared in our culture. And it could be one reason why back pain is so common in the U.S.
    As has been highlighted by research presented at the national meeting of the American College of Rheumatology, a very important aspect of the individual evaluation is the patient’s own understanding and perception of their particular situation. British researchers found that those who believed that their symptoms had serious consequences on their lives and that they had, or treatments had, little control over their symptoms were more likely to have a poor outcome. This research points out to physicians the importance of addressing the concerns and perceptions that patients have about their condition during the initial evaluations.
    Results after four years of follow-up showed that in general, otherwise healthy people who have surgery for one of these three conditions are likely to fare better than those who receive non-operative care. However, the results also indicated that people who are reluctant to have surgery may also recover with non-operative treatments if their conditions are not progressing and their pain is tolerable, and importantly, delaying or avoiding surgery did not cause additional damage in most cases. Researchers are continuing to track SPORT patient cohorts over a nine-year follow-up period to assess longer term treatment results and cost effectiveness across treatment options. In the interest of improving surgical techniques, NIH also is funding research on factors that contribute to the success or failure of artificial disc replacement surgery, including studies to compare discs on the market for significant differences in their durability rates over time.
