Men and women are equally affected by low back pain, which can range in intensity from a dull, constant ache to a sudden, sharp sensation that leaves the person incapacitated. Pain can begin abruptly as a result of an accident or by lifting something heavy, or it can develop over time due to age-related changes of the spine. Sedentary lifestyles also can set the stage for low back pain, especially when a weekday routine of getting too little exercise is punctuated by strenuous weekend workout.

Analgesic medications are those specifically designed to relieve pain. They include OTC acetaminophen and aspirin, as well as prescription opioids such as codeine, oxycodone, hydrocodone, and morphine. Opioids should be used only for a short period of time and under a physician’s supervision. People can develop a tolerance to opioids and require increasingly higher dosages to achieve the same effect. Opioids can also be addictive. Their side effects can include drowsiness, constipation, decreased reaction time, and impaired judgment. Some specialists are concerned that chronic use of opioids is detrimental to people with back pain because they can aggravate depression, leading to a worsening of the pain.

Initial management with non–medication based treatments is recommended.[6] NSAIDs are recommended if these are not sufficiently effective.[6] Normal activity should be continued as much as the pain allows.[2] Medications are recommended for the duration that they are helpful.[13] A number of other options are available for those who do not improve with usual treatment. Opioids may be useful if simple pain medications are not enough, but they are not generally recommended due to side effects.[4][13] Surgery may be beneficial for those with disc-related chronic pain and disability or spinal stenosis.[14][15] No clear benefit has been found for other cases of non-specific low back pain.[14] Low back pain often affects mood, which may be improved by counseling or antidepressants.[13][16] Additionally, there are many alternative medicine therapies, including the Alexander technique and herbal remedies, but there is not enough evidence to recommend them confidently.[17] The evidence for chiropractic care[18] and spinal manipulation is mixed.[17][19][20][21]

Regular activity and gentle stretching exercises is encouraged in uncomplicated back pain, and is associated with better long-term outcomes.[8] Physical therapy to strengthen the muscles in the abdomen and around the spine may also be recommended.[28]  These exercises are associated with better patient satisfaction, although it has not been shown to provide functional improvement.[8] However, one study found that exercise is effective for chronic back pain, but not for acute pain.     At the beginning of the data collection, participants were asked to stand in a neutral position “with their arms by their side, weight evenly balanced, and looking straight ahead”. Kinematic data were collected over 30 seconds of neutral static stance for comparison against the seated conditions. Participants sat in the office chair and in the same seat pan but with a lumbar support pillow (“Logic Back™”, Mediflow Inc., Toronto) to test the effect of the back rest profile on comfort and lumbar and thoracolumbar postures. The lumbar support is a portable device, convex in the anterior direction and contoured with an arched opening above the seat pan, that provides space for the bulk of the posterior pelvic tissues (Figure 1). The back frame of the device is constructed of a solid plastic, curved side-to-side and is relatively rigid. The frame acts as a bow which is strung anteriorly by four adjustable straps. These straps provide an elastic, anterior projection above the buttock tissues. A band affixes the device to the chair back. The lumbar support was “fitted” to each participant prior to testing by having the individual sit in the chair in a relaxed fashion, with the hips and knees flexed to 90°, feet flat and looking straight ahead. The pelvis was pushed all the way back into the aperture of the pillow, and the individual’s lumbar spine rested against the back rest. The straps were tensioned to participant preference.
