“The problem is, when you look at the studies, two years out, the outcomes from having that procedure [for a disc herniation] and not having that procedure are the same,” she explains. “Because there is a lot of rehab involved if you do have it, and the natural history of a disc herniation is that it will go away and disintegrate within a month or so, and disappear.”
When sitting in an office chair, a good lumbar back support should be flush against the small of the back. Many portable lumbar back supports are shaped specifically so that one end should be positioned up and the other down. Arthritis: The spondyloarthropathies are inflammatory types of arthritis that can affect the lower back and sacroiliac joints. Examples of spondyloarthropathies include reactive arthritis (Reiter’s disease), ankylosing spondylitis, psoriatic arthritis, and the arthritis of inflammatory bowel disease. Each of these diseases can lead to low back pain and stiffness, which is typically worse in the morning. These conditions usually begin in the second and third decades of life. They are treated with medications directed toward decreasing the inflammation. Newer biologic medications have been greatly successful in both quieting the disease and stopping its progression.
To understand various causes of low back pain, it is important to appreciate the normal design (anatomy) of the tissues of this area of the body. Important structures of the low back that can be related to symptoms in this region include the bony lumbar spine (vertebrae, singular = vertebra), discs between the vertebrae, ligaments around the spine and discs, spinal cord and nerves, muscles of the low back, internal organs of the pelvis and abdomen, and the skin covering the lumbar area.
COX-2 inhibitors, such as celecoxib (Celebrex), are more selective members of NSAIDs. Although increased cost can be a negative factor, the incidence of costly and potentially fatal bleeding in the gastrointestinal tract is clearly less with COX-2 inhibitors than with traditional NSAIDs. Long-term safety (possible increased risk for heart attack or stroke) is currently being evaluated for COX-2 inhibitors and NSAIDs.
A Grahl Duo Back™ office chair (Rohde & Grahl, Steyerberg/Voigtei, Germany), fixed in position to prevent it from swivelling or rolling, was used for the study. The arm rests were lowered so that they were not used and to ensure the maximum amount of loading was transferred to the seat pan of the chair. The chair had all the features of a typical ergonomic office chair but it was unique in that the back rest did not provide any specific lordotic support and was split vertically, providing access to the midline for sensor attachment.
At the beginning of the data collection, participants were asked to stand in a neutral position “with their arms by their side, weight evenly balanced, and looking straight ahead”. Kinematic data were collected over 30 seconds of neutral static stance for comparison against the seated conditions. Participants sat in the office chair and in the same seat pan but with a lumbar support pillow (“Logic Back™”, Mediflow Inc., Toronto) to test the effect of the back rest profile on comfort and lumbar and thoracolumbar postures. The lumbar support is a portable device, convex in the anterior direction and contoured with an arched opening above the seat pan, that provides space for the bulk of the posterior pelvic tissues (Figure 1). The back frame of the device is constructed of a solid plastic, curved side-to-side and is relatively rigid. The frame acts as a bow which is strung anteriorly by four adjustable straps. These straps provide an elastic, anterior projection above the buttock tissues. A band affixes the device to the chair back. The lumbar support was “fitted” to each participant prior to testing by having the individual sit in the chair in a relaxed fashion, with the hips and knees flexed to 90°, feet flat and looking straight ahead. The pelvis was pushed all the way back into the aperture of the pillow, and the individual’s lumbar spine rested against the back rest. The straps were tensioned to participant preference.
For many people who work in an office setting, sitting in an office chair without adequate back support can create a great deal of stress on the lower back. This is largely because in the seated position, the lumbosacral discs are loaded three times more than standing, and sitting without back support usually leads to poor posture, which stresses the soft tissues and joints in the spine. For many people, sitting in an office chair either causes or exacerbates lower back pain.

A third alternative device for achieving lumbar support is the belt. Lumbar belts are usually anywhere from four to eight inches wide. Most designs are constructed to fit easily under the clothing, making it possible to wear the belt in just about any situation. Thicker belts of this type are often employed as a safety measure for warehouse workers, truckers, and others who may be required to lift heavy loads as part of their daily work routine. The belts make it harder to strain the muscles of the lower back engaged in lifting.
The US National Library of Medicine has information on a study done to determine whether a lumbar support pillow, outfitted with a cut-out to accommodate the bulk of posterior pelvic soft tissue volume, is more effective than a standard chair in promotion a neutral spine posture and improving subjective and objective measures of comfort in healthy individuals and patients with lower back pain.
