While past authors have commonly measured comfort through subjective means , objective measures such as changes in posture (or ‘micro movements’) may be good indicators of discomfort [18-21], as small movements are necessary to alleviate pain caused by static postures. While several past studies have examined the effects of various lumbar support pads, few have quantified the level of comfort through ‘in chair movements’, and most studies have been restricted to healthy individuals. The purpose of this study was to examine for differences in lordotic curvature and comfort between a support device that accounts for pelvic tissue bulk against a typical chair in healthy individuals and patients with LBP. Comfort was measured through subjective and objective means. The hypothesis underlying this work postulates that there will be differences in comfort and lordotic angulation for healthy individuals and patients with pain between the support conditions.
For persistent low back pain, the short-term outcome is also positive, with improvement in the first six weeks but very little improvement after that. At one year, those with chronic low back pain usually continue to have moderate pain and disability. People at higher risk of long-term disability include those with poor coping skills or with fear of activity (2.5 times more likely to have poor outcomes at one year), those with a poor ability to cope with pain, functional impairments, poor general health, or a significant psychiatric or psychological component to the pain (Waddell’s signs).

Management of low back pain depends on which of the three general categories is the cause: mechanical problems, non-mechanical problems, or referred pain. For acute pain that is causing only mild to moderate problems, the goals are to restore normal function, return the individual to work, and minimize pain. The condition is normally not serious, resolves without much being done, and recovery is helped by attempting to return to normal activities as soon as possible within the limits of pain. Providing individuals with coping skills through reassurance of these facts is useful in speeding recovery. For those with sub-chronic or chronic low back pain, multidisciplinary treatment programs may help. Initial management with non–medication based treatments is recommended, with NSAIDs used if these are not sufficiently effective.
Steroids: Oral steroids can be of benefit in treating acute sciatica. Steroid injections into the epidural space have not been found to decrease duration of symptoms or improve function and are not currently recommended for the treatment of acute back pain without sciatica. Benefit in chronic pain with sciatica remains controversial. Injections into the posterior joint spaces, the facets, may be beneficial for people with pain associated with sciatica. Trigger point injections have not been proven helpful in acute back pain. Trigger point injections with a steroid and a local anesthetic may be helpful in chronic back pain. Their use remains controversial.
